Contact your pharmacy first. Ask them to send us a refill request even if there are no refills left.

Becoming a New Patient

If you’re interested in becoming our patient, please take a look at the biographies of our doctors then call us once you’ve selected who you would like to have a free meet and greet with.

Records or Documents

Please do not send any attachments. It will be disregarded.
All documents should be mailed, dropped off or faxed to us.

Emails are for non-urgent matters.

Expect a response within 4-5 business days.
